In the past five years of accountable care organizations, payment model experiments, and different methods to shift from sick care to preventative care, a variety of population health management arrangements have been implemented. In this session, we’ll review a variety of population health strategies that have stood the test of time, looking at the reasons for these programs’ longevity and financial sustainability by drawing from case studies, national program analysis, and presenter experience.
